Depends on the kind of examination--if it is filling in sqares or circles, or underlining the best possible answer, then yes, those kinds of things should be replaced. However writing exams are fine.

No. However there should also be other concurrent assessments like tutorials, unit tests
research/dissertations, seminars/ paper presentation etc to make assessments more objective and goal oriented.
