Can an employer pay for some employees insurance but not others and of those it does contribute toward base the amount on personal preference?

I suggest you check with your state's insurance commissioner. I think they can probably pay for some based on job title, like all general managers and no one else, but I wouldn't think they can just choose who they help out. But, I don't know for sure.

It may be annoying and unethical but it's not illegal unless your company handbook, employee orientation package, or any other info regarding benefits does not say it is available to all employees. It's a benefit, not part of your salary, and just like a bonus, they can give it to whoever they want, and in whatever amount they want. Although personally I would question the integrity of that employer. If they play favorites with benefits, who knows what else they are up to.

Hey, well yes the an employer can offer benefits to some employees, but if you work in the same as the other employees then you are entitled to those benefits, if you have any problems you can contact your local labor union. good luck
