I don't think there is a simple answer to this, so just some thoughts ...
If you notice your system is running slowly there could be several reasons.
Have you enough RAM and /or is your hard drive fast enough for the applications you are using.
De fragmenting a drive is about having data on contiguous areas. This you may need to do if you are adding and deleting many files to your hard - Like a high performance database.
I find the tools that comes with XP sufficient.
I do not run defrag unless I really need to. I have seen older disks on older systems lock-up, never to be accessible again - so be careful. I have a work laptop P4 2.8G Intel CPU which only has a slow (5400) small (30G) hard-drive running a .net and SQL Server development environment and it hasn't needed a defrag in two years - and believe me it gets a hammering and still performs! - but you may be running different applications.
Simply doing a disk clean up is about good housekeeping - getting rid of old files. If a files takes up a huge chunk of contiguous space then it means your disk could possibly write to this area slightly quicker. I believe it does do a bit of compressing also, but remember when your applications use compressed files, they have to decompress them before they can use them!

You can do Disk cleanup as much as you want but Defragmentation should be done every 1-2 months depending on how many files you delete.

You can do it as often as you like. Unless you add files to your system really often, do lots of downloads that create temporary files and such, it's kind of pointless to do it more often than every few days. I do it a couple of times a year, when things start bogging down. OTOH, I also have specific means for downloading files, delete incomplete downloads manually, and that sort of thing. I also send downloads to a firewire drive to keep them from bogging down C. If you practice good computer hygiene, you can get away with doing disk clean-up as often.
