DO NOT EVER GIVE A HORSE AWAY!!!!!!!!!! you will get people that do animal testing and slaughter houses coming to your house PRETENDING to be a loving horse owner and that is not good. don't take them to an auction either. put them up on dream horse for sale. or any other similar site and list the stipulations there. be careful.

I don't know about a form but you can call an equine veterinarian and get one of the vets to do a pre-purchase exam for the horse.
