Open Windows Media Player. Then Tools->Options->Privacy uncheck the History Option.

The only way to 'permantly' remove a music file is to go to the folder it's stored in and delete it there. When windows media player plays a song it extracts it from the location of the file. If you delete it from media player, it's still in it's original location, unless you delete it from there. All my music is organized in my My Music folder... good luck :-)

Close media player, delete the file and the delete it from the recycle bin
