At 21 weeks I don't think spotting is such a bad sign, I was told after I was about 16 weeks and had blood that I am less likely to have a miscarriage without a ton of cramping in your case your water breaking early, but I would ask just in case there is a problem with the placenta or something.

You should call your doctor and they may recommend that you go to the hospital to be monitored and if you are have contractions they will probably put you on a drug called Terbutaline. What you are describing sounds a lot like preterm labor. Also if you are just spotting a little they may just tell you to call if you bleed a lot. It all depends if the blood is bright red or dark. I had a lot of this and eventually they put me on bed rest in the hospital. They aren't goint to want you to go in to labor at this point so they will try to stop it with the drugs.

Miscarriage possiblity usually ends at 20 weeks. Light spotting is normal is all pregnancys and so is cramping (as long as it is not severe) they should feel like menstrual cramps. I think you are okay, but just to be sure go see a doctor. Good luck

Some of what you are describing is normal. Is the blood bright red or dark red. If it is bright red, then you need to get to the doctor's office or ER right away. If it is dark red, you should still call your doctor, but it is not as urgent. It is always better to be safe than sorry.

Call your Dr and make an appt to see him/her today. Until then, lie down, drink plenty of water, and no heavy lifting, just bed rest. Try not to stress, and just try to stay relaxed. If the spotting gets heavier, or you start to feel cramping, go to emerg.

GO TO THE HOSPITAL!!!! My sister had that at 23 weeks and was in premature labor. She had suffered through a couple miscarriages before that, so she was scared too.
It could be nothing, but its better to be safe then sorry!

If there is ever any question....go to the doctor. Pregnancy is a major thing, and with both of my daughters, any question we had we called. It is better to know, than worry if it's the right thing.
