Try using your lower gears. They are there for a reason.
At 20mph you'll need 2nd or 3rd gear (2nd for any meaningful acceleration). Any higher gear, and your engine will struggle.
You'll need at least 30mph before you can engage 4th without labouring the engine.
At 40mph on a straight, flat road with no need to accelerate (say a suburban road with a 40mph limit) you might be able to engage 5th, but you'll need 4th for gentle acceleration or slight up-hill stretch and 3rd for any sense of urgency.
