For the same reason that the big series of games at the end of baseball series is called the World Series: we, as Americans think that we are cooler than everyone and therefore are better than the rest of the world in that sport. So much better, in fact, that we call them "world" championships because the rest of the world doesn't stand a chance! (hahahahhaa! Tell that to Team USA!)

Because it sounds better that the NBA United States Champion, which also wouldn't make sense because it's a private organization and all nationalities can join provided they can play. Kind of like how the US is made up of people from all over the World.

The NBA has been around long before other countries were basketball. There were no other professional leagues. It is the same for the NFL and Major League Baseball. The world has changed and American sports have spread around the world. Sports are more international now. The championship titles remain the same for tradition purposes only.
