In roman times, a woman named Sappho ran a girl's finishing school on the island of Lesbos. Residents of the island were called Lesbians. So Lesbian meant nothing more than Austrian, Australian, Canadian, etc., i.e. "resident of". As the only thing on the island was the finishing school, all Lesbians were, obviously female. Over the course of time, this purely residential reference has come to be replaced with one denoting sexual preference.
