I never knew, but wikipedia has a very interesting article about it. According to the article, one interpretation is that "she" refers to Mary Harris Jones who was promoting labor Unions among the coal miners in Appalachia.

I rather think one of the lines of the song tends to give the game away -
"She'll be coming round the mountain when she blows"
refers to the fact that as the train is nearing the end of the mountain run and approaching flatlands, the engine driver blows the whistle to warn people of the approach of the train.
