Grandparent doesn't drink doesn't do drugs. has no history of abuse, has no record, no history of violence, does not undermine parents authority. Isn't there some sort of guide line that is set to follow or something to make visits supervised or not? I am not sure how to ask this question.

If a judge felt the visits needed to be supervised, he probably would not allow them at all

Am sure the judge should 've a good reason for granting supervised visit, if that is the case then it will put a question mark on the other parent. If my partners parents been in the same position then I would not allow my kids to visit.
May be I'm wrong............

I am not a lawyer or a judge, but i have known of grandparents whose visitation had to be supervised. This isn't always according to the ir own reputation; sometimes it is based on the assesability of the parent in his/her parents home.

The judge might think that there is some good reason to give supervised visits because they might be trying to persuade the child one way or the other.

assuming this is the USA, their is a US supreme case a couple years old, sorry do not know the case name, but court basically said grandparent's have no legal say to visit grand kids over the parents objections, assuming grandparents do not have custody

I believe it's a grandparent's right now as in Canada.
