Having one means that blood supply to the testicles has been comprimised, sometimes severely.
Sperm production on the affected side can drop dramatically. Not a big deal if you don't want kids, but if you ever do, you may find your overall sperm count is low.
Vericocoele can be surgically repaired.

Decreased fertility, although not often noticeable unless the other testicle develops a problem.

If the cause is a tumor, it needs to be addressed immediately.

Having lived with one for 34 years, I can say, not much.
