Yes it has happend and I hate to tell you who did it. Kobe did a monster dunk right in his face. Shaq saw it coming but got over just a little late.

It's rare, but it does happen on occasion. Shaq gets dunked on less frequently than any other center in the league, because of his strength, his defensive ability, and because of the fact that most of the other centers in the league totally suck (and therefore they get dunked on a lot).

Yeah, when Chris Mihm was on the Celtics in the 03-04 season he was dunking on a lot of the bigger centers, none bigger than Shaq however, who Mihm absolutely posterized.
