Just like any other business, you need to get notification out to all the patients who use this doctor, along with the hospitals or other clinics where the doctor has admission privileges.
These people need to have their records available for pickup during a certain time period so they can hand deliver them to their new health care provider who is taking this doctors place.

Before you go on this trek. My advise is to consult a lawyer. They are going to sue you.
If they are that bad call the media.
