You have two locations to concern yourself with - the Works Cited and the in-text citations.
In the Works Cited, if you have no author, you begin the entry with the title of the book.
For the in-text citation, you match the Works Cited, so you use a part of the title or the full title.

Use citation machine--it's free!. Just put the information you have in the fields, and it will give you the MLA citation.

You really need the authors name. The only reason you would not write the authors name would be if it were your primary source. Always put the author's name for a secondary source.

Yes, and begin with the next item in the format of the end note or bibliography. Remember to alphabetize your bibliography, and use the first letter of the first word of this entry for purposes of alphabetization.
