Why do people use it to refer to the woman's breast when an nursing infant is hungry? Example: When my daughter is hungry her grandpa will say "she wants ninny mama"

A ninny is a fool or simpleton. It is believed to be derived from the word "innocent."

To me the word ninny always meant some kind of insult, like idiot.

I've never heard it used the way you're describing it.
