Yes. The plaque says -- "Here men from the planet Earth first set foot upon the Moon July 1969, A.D. / WE CAME IN PEACE FOR ALL MANKIND."

the first men on the moon did leave a plaque and an american flag to mark the day we set foot.

Yes. There is at least one such attached to the landing stage of one of the lunar landers.
