You will need to speak to your probation officer about getting this permit before you leave the state. Don't go in to the probation department on the morning you want to leave and expect to walk out with your permit. You probation officer needs to approve the permit, and then it needs to be taken to the judge for his/her signature.

You will need to keep the paper with you at all times and follow all directions of your Probation Officer.

if you're on probation or parole, you are probably forbidden to leave your state without prior approval. Breaking the law can cause your probation/parole to be revoked, with a stiffer jail term instead.

I'd tell your probation officer about your plans, and seek his/her advice about what forms you need.
