Are there any specific examinations that a 30 year old sexually active gay male should undergo as part of a routine physical exam?

Being gay won't have any baring on your physical. The only suggestion I have is that you get tested for STDs. I don't say this because you're gay, but because it should become a part of every physical taken by anyone who's sexually active.

Do you mean as a result of anal intercourse? You should have a regular check up at an STD clinic if you are sexually active with multiple partners. In particular, you may contract warts inside the rectum. These are often diagnosed during a sigmoidoscopy, although a visual examination of the rectal wall may also reveal them.
If you have a single partner and practice safe sex, there isn't a need for specific exam, unless you have any symptoms.

It's not always mandatory. Usually men get these exams regularly once they reach the age of 40. It's not a fun thing to go through, but it is over quickly. You can talk over your concerns with your doctor. If you get bizarre answers on here, pay no attention to them. It is perfectly normal for any man to be concerned about his health and especially about any procedures you have to go through in a physical exam. The doctor usually gives you this pep talk about why men should get the examination of their prostate gland, and then he or she describes what they are about to do. It's over pretty quickly. Things will go alright for you.
