Yes those animals are called asexual animals where they don't need to mate with another animal to produce offsprings. Most of these are invertebrates though.

Yes animals can be hermaphrodites. Such animals are called synchronous hermaphrodites. They posses both male and female organs. Read more at the link #1 below.
SInce you also mentioned about humans, yes, such cases can occur as a result of developmental problems. There are various causes of these occuring in humans. The more interesting one is via chimerism.
When a male and female embryo fuse in early development stages to form a single embryo, 2 distinct cell lines (one cell line with karyotype XX, and the other XY) within the same person develop. Read link #2.

if u mean in situation where they are not usually hermaphrodite but through birth defect they are made one the answer is yes. Genetic defect or developmental errors are not exclusive to human. However there are also animals that are hermaphrodite normally like snail and earthworm for starter hope this helps

Yes. There are some hermaphrodite animals. These animals have organs which reproduce sperm and ova. However, the production of sperm and ova usually occur at different times, so these animals generally do not fertilise themselves but rather other individuals of the same species. Some examples of hermaphrodite animals are earthworms and leeches.
