Really need some definitions, but -
if you refer to family/friends, & if there are also professionals involved, for everyone's benefit you could make a list of contact names, & nos/ area of support provided.
e.g. Anne next door (Tel No) does shopping - calls in daily for list
Mark (son) (Tel No) calls in after work, weekdays
Put list in prominent place (by phone?) - it is essential that communication must be developed between carers.
