If he is old enough to drink, how about a bottle of wine and put it into one of those puzzle boxes. I bought one for a male friend and he loved it. You have to free it from the puzzle before you can drink it!

Throw a party with a ton of decorations. Just put streamers of coordinated colors everywhere and in different designs. Move furniture if you have to. Get large pieces of paper and paint, and put designs that go with your theme. Also, make a big "Happy Birthday" sign.
Invite everyone you know and provide refreshments. Maybe make it a costume party. Buy your best friend a crown and make a cake. If you can't make the cake get a friend to do it. Make a party play-list so people can dance, and don't forget to take lots of pictures to remember it all.

You left out some imperative details. How old is your mate? What sorts of things does he like?
Think of the things he likes...how can you give that to him?
Perhaps a DVD of his favorite sports event or movie?
How about going down to the drag strip and race your cars?

Think about what he likes doing. Could you take him paintballing or something? Or get him a box and fill it with silly wrapped up gifts that have an amusing meaning between you both....
Ask his other friends and mayb do/get something together?

think about 3 things he likes.
make a homemade card with pictures of these things on it. search the things on google images.
ive been doing this for all my friends this year and they have loved it more than the cheap presents that they get.
its personal and no-one else will have got them the same thing.

this would of been so much easier if we knew some things about him what he likes hobbies etc but for the most part if he is your true friend then he will like whatever u do for him. bar pool etc stuff like that just make it fun for the both of use or all who is a part of it.
