The new name change certificate will have both the new and old names. Otherwise how would anyone else know what your name was or even 'who' changed their name.
And you don't necessarily have anything to hide.. Some people change their name so their surname is their mother's maiden name after parents get divorced etc.

I'm not sure, but i do know that you have to use it along with your original birth certificate, not just on it's own. So certain people (ie bank staff, employers etc) are going to know what your name was before you changed it.

i am not sure if the new name will be put on a new birth certificate....go to your local town hall and enquire before you change your name....
