you can make it anyway that you would like. there is not set rules for it as far as i know.
you might be able to find a blank premade book at a craft store like Joann's or Michael's, even places like Barne's and Noble have them. That would be the starting off point.... Make it personalized and jazz it up yourself...

get a note book for a dollerstore and just wright in there .... but i have a question who not say it to there faces?
