I don't think anyone even has 3 forms of ID so I highly doubt that. Someone who has actually won a jackpot, please answer. I know it varies state to state. If you won in Las Vegas, did you have to physically present the social security card?

It was quite a few years back that I won a jackpot, but they didn't ask for a SS card. I don't even see how they could expect people to be carrying one around; it isn't safe! "Couch" was wrong, though - they take your number and report it to the IRS for you, so you can't get away with "forgetting", just the same way your employer reports your wages. By the way, the minimum win that needs to be reported is $1,200, the last I noticed, but ONLY if you win it all at once! If you win $1000 at one place, and $500 at another table, they don't pay attention, much less report it!

My wife says she believes you have to show an ID and sign a form W-2-G. This is for amounts $1200 and over.

if its over $1000 you need to fill out a 1099-G and claim that income, and yes they will ask you for a SSN. I worked in the accounting department of the Alladin filing those damn forms.

yes you will need your social security card. You will have to turn in your winnings on your income tax.
