OK, you asked for it; here's my scenario:

I have a bad feeling that the long-awaited, dreaded terrorist nuke will finally make an appearance. The event will occur in a majority Democratic city situated in a majority Republican state. A passport (slightly singed but otherwise in perfect condition) will be found floating through the air 100 yards from Ground Zero. The miraculous survival of this document will enable Homeland Security to identify the terrorist leader as a nephew of President Ahmedinejad's, who just happens to also be a colonel in Iranian Intelligence. War with Iran will begin within 72 hours of the nuking of America and will include the use of tactical nukes against hardened installations.

Most Americans will, needless to say, buy the government's explanation. Patriot Act II will pass the House and Senate with no difficulties followed soon by the reinstitution of the draft, Patriot Act III, and the creation of the NAU.

Preparations for yet another nuclear "false flag" op to enable the invasion of Venezuela and the passage of Patriot Act IV will however be interrupted by an actual, honest-to-God real attack, launched by the SCO ( aka, the Russo-Chinese alliance) who are by this point convinced that the US has gone mad and is too dangerous to be allowed to exist. The attack will be delivered not by missiles or bombers but by dozens of pre-positioned nuclear weapons which had been smuggled in years before. Because the weapons were pre-positioned in place, warning time will be precisely zero, and the attack will be a total success. The attack will end the US as an advanced economy, and as a miltary power and, quite possibly, as a political entity. Even if the USA as a political entity does survive the nuclear attack, it will be torn apart afterwards by ethnic violence, which it will no longer be rich enough to sedate with material goods or strong enough to repress with force.

North America will breakup into ten or twenty smaller countries who after a decade (or two or three) of killing each other will buckle down to the long, hard grind of rebuilding advanced societies.....

A task which will be complicated by the fallout (cultural, economic, and well.... radioactive) of the Russo-Chinese war that will inevitably follow the destruction of the only thing that had brought them together to begin with:
