Control panel, user accounts, pick an account to change, change password.

Depends on the version. For Win 98 and earlier:

1. Hit esc at the password prompt.
2. Windows will boot.
3. Search for any PWL files (search *.pwl)
4. Delete these files.
5. Windows will boot up without a prompt next time.
