how old are you this can make a difference. If you are going through puberty it can be normal due to the new production of sperm. If your underwear is to tight it can cause them to hurt. If you hold them during self gratification and pull on them it could cause bruising.

Masturbating/sex too often
masturbating/sex too little
recently a bad kick to the nuts
