The wife and daughter died and went to a replica of the living world. They and the police officer died in the separate crashes. The movie implicated at the end that they went home to their idea of heaven (or maybe just to haunt it) while the husband lives on the other side and cannot see them.
Sure makes the notion of having sex uncomfortable if your dead relatives can watch!

The director was trying to make a point that you can be on two different planes of existance and occupy a space at one time. Even though the wife and the husband were in the same room together they were on two completely different planes of existance and couldn't reach each other even though they were side by side.
