Don't Know about Canada, but one of the best experiences I had in the UK was to Shadow a Zoo keeper for a day, it invovled feeding and cleaning out the animals. It was a great day and very special and an unusual gift, not sure if this will help but good luck in your quest

From your description, your friend is obviously somebody very dear to you, that you want to get the best for. After reading your description, and thinking long and hard about the situation, I beleive the best thing you could get for her would be... a stripper. I think she'd probably love that more than anything else in the world.
Considering she lives in Canada, i'd probably go for one dressed as a Mounty, or Mountie (not sure of the spelling). If i was her, i know that's exactly what i'd want to brighten up my birthday.

Take her to a place she's talked of visiting but never get around to go to. A special art exhibition, museum, a new restaurant with that special sort of decor she likes. My husband knows that I am very fond of Shakespeare, and invited me to Stratford, just to see the sights and enjoy a good dinner (He detests Shakespeare!).

How about a girls weekend somewhere or just a girls day. Go to a spa. It'll be a treat for the both of you. Does she have a favorite artist...buy one of their portraits. Does she have a favorite type of music or musician...take her to a concert.
