Go to an English speaking country and hang around the people... Without trying, you will pick up grammar and some new words when they talk about things. You also won't be learning 100% proper English like they teach you, but you will be learning the way people actually talk, and you will learn slang too. If you can't go to another country, read some English books and watch movies in English with your native language subtitles so if you get confused you can still know whats going on Luck

A good vocabulary is central to good spoken English. Although the effort takes time, I believe there are no short-cuts to spoken English for foreigners.
I used to take Enslish class for exchange students; the only difficulty they faced was in grammar, and English accents. Standard written grammar in English is very different from what native speakers speak, we take far too many liberties with speech so we have numerous accents in the English speaking world, and many people may speak in a way that foreigners find terrible at first glance. Very few Americans, native speakers them, would identify with Irish English. So, watching American TV soaps (although don't BELIEVE in their ideas), movies (again watch with caution) and conversation with US toll free number agents are starting points to improve confidence in yourself, you need to expand your vocabulary too.
I once used a book called "Six Weeks for a Powerful Vocabulary: by Wilfred Funk. I think I should highly recommend it to you. Even native speakers suffer from a shortage of good words, and they should also read it.

An excellent way to improve vocabulary is to play Scrabble! You can enjoy the game and pick up new words as well, so it won't seem like you are doing 'work', it will be a breeze :)

Over there, you should play the dictionary option which let's users play only valid words in the dictionary. Thus you can observe games or play Scrabble games yourself to improve your vocabulary.

As musie said,first step is start thinking in english....
There was a friend of mine whose spoken english was not very good. Once she asked me to help her. I agreed and told her I wont answer her if she speaks in anything other than english. She agreed and she also gave me the authority to correct her whenever she is wrong even if that is in public.
She threw away her shame of being told she is wrong and learnt from it every time I corrected her!
You may try that with a person, you know who speaks good english....
And of course, from now onwards, again, as musie said earlier:
"Think In English"....
Try to figure out everytime if whatever sentence you are thinking should be the way you are thinking or someone said it in a better way. Try incorporating it, when you say it!

The one must for you is to read English newspapers daily and make a copy of difficult words so that yu should not roam around finding the same word in the dictionary again and again.

Read English poems you will get more new words and use dictinory to cheak the meaning of those words specially read Sheakshphers poetry.

read a lot eng.
memorize some words and try to use them in day time
listen to eng (BBC / CNN), watch eng movies
try to grasp theirpronunciation and use same asscent in ur speaking
