Any shoe shop will be able to do this for you.
Alternatively if you are near any saddlers or horse riding shops they will also have a tool that you can use, called a leather punch.

You can get a leather hole-puncher yourself in any do-it-yourself place, or a good ironmongers/hardware shop. They are inexpensive. They look a bit like a stapler with a rotating head for different sizes of hole.

Pen knife, kitchen knife, ice pick - or, as someone else said, McDonalds! I used a knife on a belt once, so would say DIY.
