Advertise it on Sedo.com and Ebay but in your listing on Ebay make sure you state that the buyer has to pay the transfer fees for the domain name, I think its around £10 upwards depending on where you bought the domain from in the 1st place. Its not difficult at all just be sure to read the on-screen instructions clearly. If its a .co.uk domain the transfer is usually free but .com and .eu there is a small charge as I have said. Have a look at the site where you bought your domain from in the 1st place and there should be some advice there that you could read. Regarding hosting, its up to the buyer to decide where they want the site hosted, they can either use the one you are using at the moment or move it where they want it, up to them.
Oh, I forgot to say that if you advertise on Sedo, you can earn commission from them at the same time so get adding!

yes

you can sell the domain name, the copy (which is intellectual property), the client list, the whole lot, together or in bits.

it's tricky to pass on the hosting package. It might be simpler just to transfer and let the geeks sort out Domain Name Services and all that cráp.

i sold a domain name on ebay a while ago.
