My name is aijaz and i am fresh web designer working in a big company i have designed one intrest form. but i don't know the script . the form in which user fill and press button.whoes information goes to the website persons or boss email id. can some body plz send me information regarding this is my emai id [e-mail].

You are talking about a feedback form. Here is a website that uses a wizard - you tell it what you want and it makes the code for you, then all you have to do is upload the code and a file to your server:
