I quit in 1987, but it was almost always my left until then. That was a time when smoking in an office, at ones desk, was allowed. So I would write or hold the phone with my right hand, and hold the cigarette with my left.
Sometimes driving, I used my right hand - I lived in England, driving on the left, right-hand drive - I used my left hand on the stick shift & helfd the cigarette in my right hand, on the wheel. With my driving - evey passenger needed to smoke, to calm their nerves :-)
