They would give you looks of disgust then
give you an enema that costs lots and lots of money.
It would be as if you just ate three bowls
of fire and lighten' chili...and it wanted out...fast.
Personally, I'd rather do the five dollar enema
in the privacy of my own home, wouldn't you?

If you are referring to a proctoscope or a colonoscope you do indeed have to cleanse your bowels. Normally that is done with some very strong laxatives the night before not an enema. If one failed to follow the procedure the Doc would probably send them home. Such procedures are to try and find colon/anal cancers or pylops and missing one can be fatal.
Now if you are only talking about a prostate exam then no preparation is necessary as the Doc uses rubber gloves and KY.

No, you don't have to give yourself an enema before a basic exam. I know because I had one and they did not instruct me to do that.
However, a colonoscopy or other tests where you need to be clean, an enema (or more with the colonoscopy) is necessary.

In the case of my sister-in-law she just lost the money for the appointment because she wasn't prepared. They refused to do the exam and she was charged.

They ask you to do that so they don't have to work around all the feces left in your rectum. It helps to keep the examination cleaner and have less odors. Make it easier.
Good point of advice...you should really always do what your doctor recommends...most of the time anyway

When they went to put the equipment in it would be all gross and they would know you didn't do it and they would deny you service and send you home.(and probably charge you for the visit anyway!)

the doctor would very likely be displeased by the results of having your feces on his gloved hand. It would just be gross.
