You could say something like.....
I understand the importance of exercise for at risk groups who may be referred for exercise by their GP.
I understand the importance of developing personal exercise programmes tailored to suit the individual's needs, based on their baseline fitness and background health problems.
I am willing to communicate with the GP as necessary, both with regard to the progress of the people referred, and any problems that may arise.

It might be a good idea to do out sample exercise programmes you might think would be appropriate for the different risk groups that might be reffered - i.e. someone with high blood pressure, bearing in mind that isometrics may have to be avoided; or showing your awareness of cardiac rehab programmes that you could help supervise for people with heart attacks etc.

You could also show willingness to learn about the conditions that are likely to be referred, and to train up so that you can provide them with the best help possible.
