Car B would put out 100s times less emissions than car A.
Example. Motorcycles have smaller, more fuel efficient engines than those in the typical automobile. But a motorcycle spews out much more pollution because no motorcycles have emission controls.

Not necessarily, since you have given us no information. If car A is a 4 cylinder Honda with a fouled plug and a bad catalytic converter, and car B is a well tuned new Cadillac with an 8 cylinder, car A is more fuel efficient but a polluter, while car B is much cleaner but can't get past a station without stopping for fuel.

More carbon monoxide per hour or per gallon? Not necessarily. More carbon monoxide per mile? Probably, as the the more fuel efficient car will get further with the same amount of fuel (and thus the same amount of carbon monoxide). However, carbon monoxide emission also depends on other factors, so it would depend on the specifics of the cars in question.

No. If a car is more fuel efficient, then it burns fuel more efficiently. In chemical terms more products, less byproducts
fuel + oxygen -> CO2 + water + heat
A less efficient car doesn't burn fuel completely so you get
fuel + oxygen -> CO2 + water + heat + CO + etc
The less efficient burn = more incomplete combustion/reaction
Other factors: catalytic convert, quality of fuel
