a doctor does not need to be addressed as a "doctor X" if you're their spouse, family member or friend. if they tell you to call them by their first name, then that's fine too. but if you're their patient, you should probably call them dr. x even outside the office/hospital situation unless they specify otherwise. as a nurse, you should probably also call them dr.

When they ask you to call them by their first names. I work with two doctors, one would have us call him by his first name all the time, but it's against office policy. The other would rather everyone call him Dr. I can see where he's coming from, they work very hard to become doctors.

when you yourself are a doctor or when the doctor says call me XYZ. but always introduced as doctor XXX YYY, no longer mr or ms/mrs
