all depends on whose birthday it is.
If it's your boyfriend or husband buy a pair of crochless knickers, a baby doll nighty and a good perfume. Wear it after the kids have gone to bed and give him a smile and a wink.
If it's that old bloke next door - best not do that - you don't want to be accused of killing the poor old fellah!

well, many good answers here. but just in case you not pregnant during the time birthday happens and dont feel like making love either, my best gift would be a rich creammmmy cake with a beautiful message on top!
