Man, as a species, is infinitely inventive.
It doesn't matter how many ways people come up with to make something foolproof, there will be somebody who finds a way to confound it.
It is because we all respond to any given situation in a unique way. Ten people share the same experience, e.g. witness an event, hear a conversation, or read a set of instructions, and each of them will give you a marginally, (or sometimes wildly) different interpretation of what they have experienced.
