Rafters hold up the roof and ceiling joists are the boards to which the ceiling is attached. Unless you have full cathedral ceilings or post and beam construction. Then they're one in the same.

Rafters hold up the roof tiles, and joists hold up the floors

Floor joists are just that - they have the downstairs floorboards on them ( if you don't have concrete floors!)

Chamber joists carry the upstairs floorboards, and also have the downstairs ceiling nailed to the bottom of them

Roof joists carry the upstairs ceiling, and have the rafters nailed to either end of them ( imagine a triangle) - you can see that in any loft

Rafters are the Roof Frame, Joists are used to nail Sheetrock to, but also hold the walls plumb. Sometimes there are no joist and sheetrock is nailed to the Rafters, like in an A frame house.

Ceiling Joists are the horizontal timbers that run across between the tops of the walls.
Rafters are the sloping timbers that run up the pitch of the roof.

The top floor insulation is either at the roof or the first floor ceiling, (floor of the attic), not both. If it's already at the ceiling, the attic is vented to the outside, and insulating the roof would make very little difference. Laying an additional 'blanket on the floor is the way to go if you want to upgrade. Insulating a roof increases the summer temperature of the roofing, and might adversely effect the life of some materials.
