Does the poor granny have to sit naked in the tub while the thing fills up and drains? Wouldn't opening the door to get in/out with the bath full of water let all the water out?

Yes she does.Make sure she has a thermostatic bath filler with a fast flow rate.
They empty with a twin waste, make sure the plumber uses a large (4"plus) pipe for for fast emptying.

There is usually a black line to fill the bath up to at the correct temp, the person sits in, the water comes up to just below knee high. The rubber sealed door is closed and made water tight. Then the bath which is like a bucket seat can be tipped back, this makes the water run underneath the body and the level can then be topped up.NOTE pull the plug out and drain some of the water before you tip it back to the seated position as failure to do this can create a mini tidal wave over the floor also remember to make sure the person is kept warm with a towel whilst waiting for top half of the bath to drain down to knee height again then the side door can be opened with out water spilling on to the floor

They have to climb in and shut the door (which has a special rubber seal) then fill up the tub.

Too bad for granny if the water is too hot or cold.

She would be better with one of those lowering bath seats that lift you up to the top of the bath level or lower you into the water.

have to sit in the bath when filling and emptying ,if you did open the door when the bath was full, you would flood the bath room ,think would be difficult to open the door when then bath is full because the door opens into the bath ,think the volume of water in the bath would prevent the door opening, a walk in shower ,(shower floor being level with bathroom floor ) is a lot easier for the elderly to use.
