Some transvestites do go out in public and apparently try to pass as female. Some are quite obvious. Some are completely feminine in appearance.
However, that said, there are some distinct indicatiors. Adam's apple is a clear giveaway since females do NOT have one. But this is not always the case as in a man it may not be visable, but will be felt by touch under the skin. Hip joint width - women have wider hip joint spacing than men, for giving birth, which also means they have a distinctive gait which rolls the hips more than a man of the same size. The wider hip joint placement and hip rolling gait is why women can walk in high heels. The whole pelvis is shaped differently, men narrower, women wider. Shoulder width to hip width ratio - men typically have hip joints directly below or narrower than the shoulder joints, whereas women are typically narrower at the shoiulder and wider at the hip. Body hair - men typically have facial hair which is coarse and women do not, women have a very fine peach fuzz type if any at all. However, there are methods of laser hair removel which can prevent a beard from growing on the face. I knew a pre-op transexual, who only had the one final operation left to go. She had her voice box shaved and tightened to raise her voice pitch, her adam's apple shaved to make it invisible (but it was still there and could be felt under the skin), laser hair removal on her face and neck, and also breast and hip implants to give her the right shape for a female figure. She had the perfect feminine 80-70-100% ratio of bust to waist to hip measurements. Had she not told me, I would have never known, since none of the usual male/female indicators were apparent.

hmm....are you asking about a transvestite? If so, just study how the forearms line up when seen palms up. Women's arms tend to veer out whereas men's tend to stay linear.
There are some folks who have genital defects due to DNA mess ups. It certainly not a question that I would ask unless you are far along in a relationship. It can be considered as"sexual harrassment" especially at work subject to severe discipline and/or lawsuits.
