Both facilitated diffusion and active transport refer to molecules aiding other moelcules passage across a membrane. However, facilitated diffusion is movement of a substance across a membrane along the concentration gradient (ie...something goes from an area of high concentration to an area of low concentration...like the spreading smell of baking cookies) whereas active transport often goes against the concentration gradient, acting like a pump bringing something across a membrane even though it may already be in high concentration already. Active transport will often occur across a cell membrane to bring required nutrients into a cell.

Both facilitated diffusion and active transport require a protein in order to allow the molecule to pass through.
