I think that we need more details to answer this question fairly, because it all depends upon what you did and how old you are. If you are an adult then ultimately you probably didn't fail him he is just disappointed in you. You need to explain to him that you love him very much and you didn't mean to do what you did depending upon what it is and we are all human and we all make mistakes too! Let him know that you still love him and you know that he still loves you a parents love NEVER goes away! They may become disappointed in you but they will NEVER stop loving you! Do something nice for him like take him out to dinner as father and daughter and have a nice chat with him about whatever it is that went wrong. Make sure that you tell him you love him tonight!

Did he tell you that, if so maybe there is a reason why you aren't allowed to see him. Putting that kind of responsibility on your chidren is just not fair.
Parents love their children unconditionally maybe you just think you failed him but I am sure if you were to discuss what you think you did, you may be surprised at his answer.
If your dad has been ordered by the courts to stay away from you this is a different matter, you see, we assume the worst because your question doesn't give much detail.
Once you are of legal age you can try to find him, and he you.

well if you are happy where you are in life then you should tell you dad that you are. He should love you no matter what and there must be a good reason why you dont see him. tell him what you think and how you feel openly and honestly.

You don't see him???? Is it you or him? If it is your decision and you want a relationship with him, go for it. If it is his decision not to support you emotionally and see you, then he ought to be ashamed of himself. We love our children unconditionally.....believe me when I say I have watched 17 years from a child with horrendous decisions made....do I love him less????? a big NO...just talked to him a few minutes ago as I do everyday...Go ask him why he thinks you have failed him and I don't care what his answer is....ex. drugs, alcohol, sex , baby...nothing should keep a parent from loving their child....
