Just send her a short message saying hi and briefly tell her what you've been up to and ask how she is doing.
Don't go into too much detail, and don't get into what happened in the past. She'll be more likely to respond if she knows you aren't going to grill her for not talking to you or get all personal right away.
If she does respond, great! If she doesn't, that's fine. You've survived without her for five years, there's no need to be devastated. Don't look at it like you'll be crushed if she doesn't respond. Just look at like you'll be delighted if she does respond, but if not, that's ok, too.
There are plenty of people out there who would love to have a good friend! Don't waste your time and energy on people who have moved on. Regardless of whether she stays in touch this time, get out there and make new friends!

You should give it a try. What can it hurt? If she doesn't respond don't worry about it maybe she has gone through some things in the past 5 years. Don't take it personally

Sometimes when people move they get caught up in their new lives and tend to loose old connections. If you were best friend I would definately contact her. I bet she will be really glad to hear from you and tell you all her new stories. If not then at least you tried. Couldn't hurt.

As long as it doesn't interfere with your life at the present. I had a problem like this. It didn't really benefit me nor did it harm me. My life has already moved on. My past relationships with people are not the same. I hope I helped you out some.
