Anyone seen an example of a bad form on the web?

I am doing a research about forms on the web. I need examples of Good one and Bad ones. Some forms are easy to fill other s are frustrating. If you experienced a bad form please inform me.

There are a lot of them that require you to enter your telephone number in a specific format like nnn-nnn-nnnn when that is something that their programs could be dealing with. I get so mad at them that I don't keep track of which have this common flaw but I think Coca Cola was one.
