A gut punch is something that completely floors you. It may not be the most painful thing that can happen nor the most permanently damaging but its suddeness and precision on a weak point make recovering from it very difficult.
A girl dumping you hurts. One dumping you with no warning right before a big couples event, ie. a wedding (especially yours), prom, formal dance or something else, is a gut punch.

I agree with a punch to the gut.
But it could also mean "punch" (alcohol?) that upsets your stomach.
