I had cited it in my document in a footnote. But do I need to note it in my reference page?

Yes, you do. Each style (MLA and APA) differ a little. Here's a link that will help you make the proper citation. You just select your source in the left-hand column, fill out the table and submit. It will automacitally generate a proper citation for selected style.

Yes, it is a refrence that you cited, regardless that you already cited it, it needs to be at the end. This is especially true because most professors read the bibliography to determine the grade, so they'll be looking for it.
